Home Care Service

CDPAP

CDPAP is a Medicaid program under the Department of Health NY, which provides services to chronically ill or physically disabled individuals who have a medical need for help with activities of daily living. Recipients have flexibility and freedom in choosing their caregivers.

His or her doctor must send a completed Physician’s Order for Services.
Completes a social and nursing assessment for approval.
A nurse assessor then recommends the amount, frequency and duration of services.
Last step is his or her plan need to transfer to MLTC.

Who is Eligible for Home Care Service?

Any patient, who needs assistance with day to day activities or skilled care, can apply for the Home Care Service.
To get benefit consumer must:

Have Medicaid
Live in New York
Need help with day to day activities

Who can work as Caregiver?

CDPAP encourages individuals to enjoy freedom and independence of choosing their own caregiver. Daughter, Son, Relatives, Friends, Neighbors, Existing home attendant, Loved ones- Anyone can be a caregiver except Legal spouse, Designated representative, or health proxy, Parent (if the patient age fewer than 21).
